Length of stay is rarely a hospitalist problem. More often, it reflects dozens of small operational barriers that accumulate throughout a patient’s hospitalization.
Admission timing. Diagnostic efficiency. Consultant engagement. Discharge planning. Utilization review. A delay anywhere in the chain adds time—and the time adds up.
Before recommending solutions, we identify where those barriers actually begin.
That means looking beyond our own service line—understanding how patients move through the ED, how consultants participate in discharge planning, where documentation creates delays, and where established practice patterns may no longer serve the patient or the hospital.
Then we work directly with the physicians and departments responsible for those decisions to change them.

Length of stay affects bed availability, throughput, staffing utilization, reimbursement, and the financial performance of nearly every service line.
Yet it is one of the hardest metrics to improve because no single department owns all of its drivers.
We don’t manage LOS by explaining yesterday’s number. We identify the clinical and operational decisions creating unnecessary days and build sustainable processes around them.
Length of Stay Reflects the Health of the Entire System
Within the first seven months after assuming leadership of the hospitalist program, CVCCM reduced average length of stay from 4.24 to 3.79 days—a 0.45-day, or 10.6%, reduction.
We began immediately—aligning physicians and residents around earlier discharge readiness, preparing patients the evening before whenever clinically appropriate, standardizing discharge workflows, improving handoffs, and removing barriers that unnecessarily delayed patient progression.
Because improving length of stay isn’t about pushing patients out sooner. It’s about eliminating the reasons they stay longer than they need to.
